- Online: You can make an online appointment here.
- Phone: Call our office at (305) 274-8779 during regular business hours.
- In-Person: Visit our office at 9950 SW 107th Ave STE 101, Miami, FL 33176 to schedule with our friendly reception staff.
To ensure a smooth first visit, please bring:
- Insurance Card: Your current health insurance card.
- Photo ID: A government-issued photo ID, such as a driver’s license.
- List of Medications: A complete list of your current medications, including prescriptions, over-the-counter drugs, vitamins, and supplements.
- Medical Records (if possible): If you have recent medical records from other providers, please bring copies or arrange for them to be sent to our office prior to your appointment.
Here are ways to request prescription renewals:
- During Your Visit: Discuss refills with your provider at your appointment.
- Online: If you use our patient portal, you may be able to request refills online.
- Phone: Call our office during business hours and follow the prompts for prescription renewals.

Yes! If you need specialized care, we will work closely with you to find the right specialist and seamlessly coordinate your care. We’ll handle all necessary referrals and ensure effective communication between your primary care team and any specialists involved.
Our office hours are:
- Monday – Thursday: 7:30 AM – 4:30 PM
- Friday: 7:30 AM – 11 AM
- Weekends: Closed on weekends
